The Care and Discharge Planner is responsible for coordinating and implementing care and post-discharge plans/instructions under the direction of licensed Social Service staff. The Care and Discharge Planner works with the patient, family and licensed Social Service staff to provide optimum care and discharge plans based on patient needs. Assists with staffing and completion of treatment plans. Prepares Social Service discharge documentation and prepares documentation for the next level of care providers. Sets discharge appointments and arranges transportation as necessary. Tracks and prepares Medicare Discharge letters for patient signature as needed.

Qualifications:

- Bachelors degree in Behavioral Health or Associates degree with 3 yrs experience in behavioral health.

- Prior experience in discharge planning and insurance plans preferred
