Discharge Follow Up Specialist
Under the direction of the Director of Case Management or Generations, coordinates and implements the function of discharge planning for inpatient and outpatient services. Identifies and anticipates discharge needs. Communicates and collaborates with the multidisciplinary team through verbal and written communication. Responsible for the integration of the patient to the home or transitional evironment by developing, planning, implementing, and evaluating in accordance with current existing federal, state, and local standards. Adheres to confidentiality policies specific to communication, patient confidentiality, record keeping, and coordination services. Responsible for data entry in all areas of discharge planning, including denial, patient date, and any required documentation.
High school diploma or GED. Minimum 2-4 yrs healthcare experience. Experience with Case Management preferred. COAP and CPR
