l. Position Function:

Provides direct and indirect patient care

Supervises non-licensed personnel in the acute care setting .

Communicates with coordinator, physician, charge nurse and coworkers as appropriate about changes in the patient clinical condition including results of diagnostic studies and symptomatology.

Able to respond quickly and accurately to changes in patient condition or responds to treatment

(I. Job Relationships

Provide supervision to the following staff by job category:

-Behavioral health technicians

-Unit secretary

Serves as a resource to all team members

Ill. Authority

To communicate pertinent information to charge nurse physicians and coworkers.

Consults with physicians, coworkers and departments to provide for an interdisciplinary approach to meet patient needs. ,

IV. A. Responsibilities/Essential functions:

Completes all assessments related to admission of new patients within designated time frames. Updates the treatment plan as needed based on patients identified needs.

Formulates accurate problem list and develops treatment plan after review of assessment data. Develops and updates the treatment plan as needed based on patient's identified needs.

Performs and documents accurate assessment data throughout the patients stay. Documents subjective and objective assessments for each active problem.

Identifies patient learning and educational needs during admission and throughout patient's stay

Initiate interventions and communicates with patient/ significant others to facilitate delivery of the plan of care

Evaluates effectiveness of all interventions and documents appropriately in nursing notes each shift. Documents effectiveness of all PRN medications given and specifically documents effectiveness of oral pain medication given within one hour.

Contributes to the multidisciplinary staffing process via written or oral communication. Completes assigned portions of 72 HR and weekly assessments. Ensure staffings are signed and dated by physicians, patients and significant others as appropriate.

Facilitate patient transfers and or discharges following hospital policy and procedures. Resolves treatment plan problem list at discharge. Complete transfer transport forms in accordance with policy.

Completes medication reconciliation at time of patient discharge or transfer. Consult with physicians to ensure accuracy.

Exhibits knowledge of medications that administers medications safely, accurately and on time period uses approved patient identifiers and scans wristband and medication prior to administration.

Functions as a patient/ family advocate. Maintains ethical behavior and patient confidentiality.

Addresses patients complaints and requests in a manner that conveys caring and preserves dignity.

Exhibits compassion and competence in performing service recovery .

Contributes to effective unit organization. Supports team concepts and maintain professional, positive working relationships with all members of the health care team

Demonstrates use of critical thinking skills in managing a patient crisis/ emergency or to prevent deterioration of patient's status.

Demonstrates ability to accurately assess patient acuities. Completes worksheets within designated time frames.

Floats to other units as assigned and demonstrates ability to perform nursing functions to meet patient care needs.

Functions as a clinical resource for staff. Provides grievance to psych techs and support staff.

Responsible for overall patient satisfaction as measured by the patient satisfaction tool. Completes patient assessments away from nurses station and inures privacy

B. Responsibilities/Non-EssentiaI Functions:

Performs related duties as directed by Charge RN/Supervisor/Nurse manager

V. Reporting requirements :

Reports to Nurse manager

Reports serious incidents and or severe breaches in the quality of care to CNO, hospital president, Risk manager, or Quality director.

Accountability:

Demonstrates ability to be flexible, organized and prioritize patient care activities.

Demonstrates knowledge of infection control policy and procedures.

Functions as a patient/family advocate, maintains ethical behavior and patient confidentiality.

Demonstrates use of critical thinking skills in managing a patient crisis emergency or to prevent deterioration of patient status.

Vll. Qualifications:

Minimum Education:

Graduate of an accredited school of nursing

BSN preferred

Minimum Experience:

One year experience preferred

Minimum skills/abilities:

Proven experience and leadership ability in management and clinical practice.
